I've had this bug for a few years, on MacOS ventura Apple Silicon, where randomly a box element is persisted to the top of all windows in the OS.
It's doesn't matter which app is loaded, it still fixed, and on top. I only realised the bug was coming from Fork, when I it disappeared when I close the app.
