Submission History Module blows up my form #684

When I turn on the Submission History Module for a private form, I get a 500 error when I try to enter data for the form. I have it turned off, but that could become an issue for me in the future.

Any input would be appreciated.

hmmm run System Check module and see iff give you erros.
