How to access database to show status of particular field in form by PHP/html page #825

I am using formtools 3.0.20 I am not much know about coding.Could any one help to how i connect to database by PHP/html page to show status of particular field in the form.The filed id is entered by user on PHP page and we want to show status of same field value on php/html page
