I have the exact same problem.
The mysql tables are generated, so it looks as if step 3 worked fine. However, as above, the setup doesn't proceed to setp 4. No error messages.
Hopefully somebody can help.
I had a look at the code. This worked for me:
Having performed step 3, type the URL https://www.yoursite.com/formtools/install/#/step4 in a new tab (assuming the folder you installed on is called 'formtools'). You can proceed with the installment there.

This worked for me too!
In step 3 the tables are created, but after that you can't go to step 4. If you continue, Form Tools complains that the tables with the chosen prefix already exists and you get stuck in a loop.

Also encountered this bug and the solution here worked!
Finished creating the tables. But it doesn't take me to step 4.
Per the help files, step 4 is to create the config.php file. The config.php file content is generated by the installation script.
I am pretty good at figuring things out... but I am tripping over this. Does anyone still help with these?
