adding module - -refresh fail #7

Copying and editing (as instructions) the folder 'submission_accounts' module in my /var/www/html/formtools/modules/ the refresh button on formtools modules show me an error.

I think I may see the problem. I'll release a fix for the issue I see, but without further info I can't confirm it's the same issue you're seeing.

I already upload the module to file folder but it not in form dashboard eventhough refresh. Please explain
