Update wiki for new FortifyBugTrackerUtility 3.0 release

• Replace all references to FoDBugTrackerUtility with FortifyBugTrackerUtility
• Replace all references to version 1.0/1.1 to 2.0
• Add information about automatically loading application versions/releases, and generating mapped (bug tracker related) context properties, i.e. generate Jira Project Key based on attributes or static mapping
• Restructure Wiki to describe both SSC and FoD integrations
• Update bug tracker specific pages to reflect any changes, and describe both FoD and SSC usage
• Update the 'build from source' page

As documentation is now included in the distribution zip file, and most pages have been deleted from the Wiki, this issue is no longer relevant.
