Redesign actor transformation application

The current actor transformation interface is a complex and confusing interface. With some changes coming to Wild Shape in the 2024 rules and the move to V12 this would be a good chance to rebuild that interface to be more intuitive.

• Display the token of the actor being transformed and the actor they are being transformed into, to make it clear what is being changed
• The "Wild Shape", "Polymorph", and other preset buttons should be states that update the checkboxes to make it clear what is happening
• Fieldsets should be used to group options together rather than just presenting one giant list

Would strongly like to see a "Wild Shape 2014" option retained even when the new rules are implemented. Fully support seeing the system move to the new ruleset, but as you write above, it's complex, and putting in the 2014 options manually will be a pain if there's no (customisable?) preset.
