Make case name obvious <h1> title on case pages #39

The obvious <h1> of case pages are currently the names of the courts, not the cases, which isn't very intuitive.

Upload time for all ~96621 modified judgments with rsync was ~30min.

Hopefully this will help with the Google linking issue too.
