Tested on Firmware 1.0B. The white background behind the Sega logo when starting the game doesn't fill the whole screen:

I confirmed it fills the whole screen on a real Game Gear:

I can confirm that as of the latest firmware (1.1 - 5/4/23), this issue does not occur in the openFPGA GG core. I see that OP was using a cartridge, though...
