Improve links in the editor #393

Feedback from Abbey:

The add a link button works well if you know how to use it - but I'm not sure it'll be clear that you need to highlight the text you want to link and then enter the URL in that box, etc. It'd be easier if, when you highlighted the text, that option came up like it does in Ghost. Or, I think we need to find a way to make it clearer that the button is how you add a link (unless you're using markdown).

I think we need to be able to see where there's a link in the draft without having to hover over the words that have the link, if that makes sense. Like Ghost adds a blue underline. That way we won't miss links that authors may add (this is important, in case they try to add an affiliate link or something - we need to be able to see these in the draft before we publish).
