Help identifying reporters #163

After extensively inspecting the westlaw and lexis citation dataset, i couldn't find any information from some reporters, i hope someone can help me identify them.

This is a sample list of citations where i was unable to identify the reporter

94-1781 (La. 7/6/94) (I believe these are docket numbers)
9426580 (La.App. 2 Cir. 1/25/95) (I believe these are docket numbers)

P.U.R. 1932A 522
1 Pack 255
1984-86 SERB 458
