Set extra.format for proportional bitmap fonts too #21

vqt_ext_name can now check the flag and avoid writing intout[34] twice

I noticed that proportional bitmap fonts weren't being reported correctly by vqt_ext_name(), partly down to extra.format being set too late, and partly down to the vqt_ext_name() assembler needing an update.

Tested with the ATSS/ATTR/ATTP fonts from Atari shipped in e.g. Papyrus, as well as my usual bunch of TrueType fonts.
