Support icons from web addresses #65

Reported by sgallagh on 25 Jun 2015 12:17 UTC It's inconvenient needing to retain a copy of the icon in the camera roll for iOS. It would be best if FreeOTP supported a (cached) URL option for displaying icon images.

It would be great to specify in the URL that generates the QR code the URL for the icon as well so that the entry gets added with the appropriate icon when FreeOTP scans the QR code.
