Changing structure to register field from plugins #10

The new Kirby version 2.3 brings field registration through plugins with it! With bit restructuring, you can now add this plugin as a simple submodule in your project.

Updated also the readme file.

I'll hold off merging this until 2.3 stabilises a bit.
