Rename gem from "jsontableschema" to "tableschema" (#22) #26

fixes #22

I renamed:

gem from tableschema to jsontableschema
classes namespace from JsonTableSchema to TableSchema

I also mentioned the rename in the README and described steps to transition the code from one gem to another

@roll For some reason coveralls seems to be stuck with the name jsontableschema-rb:
