Use a template for reporting errors #46

Reporters must provide the following information (in addition to a minimal non-working example):

friendly-traceback: 0.7.34 friendly: 0.7.8 Python: 3.10.5

This should make it easier to reproduce the error and to determine if the problem is present in a certain version of friendly or friendly-traceback.
