update SURFconext docs/implementation #1

The SURFconext/SAML documentation and implementation needs a serious update

use persistent NameID to identify users (+ check that you actually get this!)
use displayName for name in conference
use schacHomeOrganization as filter for restrict_create
require displayName as attribute, optional schacHomeOrganization
