Use new style hooks, config file & persistent template directory. Refactoring of code for PEP 8 & 257 compliance. Somewhat improved documentation (readme & docstrings).

Thanks for your contribution. I'm very glad to see this is still useful to someone. However, I don't use anki at all anymore so I'm unable to maintain this. Feel free to fork and maintain your own version and upload it to the anki addons site under your name.
