Hello there, I've created a polish locale translations for fulcrum, also I've added missing translations on story forms to have 100% of application translated.

Hey, It was my first pull request on github, I closed it because after Travis failed, I realized that I didn't checked tests. I will fix tests for these little changes in js, and then I will probably need to create another pull request or reopen this one. I hope I didn't make a mess.
