Attachment to note implemented via carrierwave gem and backbone-model-file-upload

I use fulcrum agile in my work and was really missed file uploads. This is draft implementation of upload files to story notes (not fully covered by tests and not fully tested). This is unfinished pull requests and I'd like to discuss this feature with the collaborators.
