Make sure that duplicates can't be added to playlist

What steps will reproduce the problem?
1. Click menu
2. Tap All Programs
3. Tap Car Talk
4. Long-press on the first item to start it playing
5. Tap the "add all to playlist" button

What is the expected output? 
The playlist should contain all items in the program, one entry for each

What do you see instead?
The first item is duplicated because it was added with the long-press and then 
again with "add all"

Please use labels and text to provide additional information.
If an item is already in the playlist and has not been played and is being 
added to the playlist, then don't add a second entry; leave the first where it 
is.

If an item is already in the playlist and has been played, how do we handle 
this?
