Having learned that San Francisco uses their own form instead of the General Petition Form, implement that form.

Outstanding questions:

Do we have testers in San Francisco? @fureigh will follow up with one potential community organization. Also worth asking Clean Slate directly or attempting a reintro to the PD.

Incidentally, the Clean Slate form is exactly the same, but with the following additions:

Are you currently...
On probation or parole? (Yes/No)
If you are on probation, where and until when?
Serving a sentence? (Yes/No)
Charged with a crime? (Yes/No)
Have you EVER been arrested or convicted of a crime OUTSIDE of San Francisco? (Yes/No)
If yes, list all the dates:
