Consent for HSA to check income in federal hub (checkbox)
If elderly (65+) or disabled, then requires verification of resources (bank statement) [come back to this]

Changing "Sex" to "Gender" per the Medi-Cal application.

"Filing status for prior year taxes" doesn't exist in the PDF application I've seen, so I'm using "Marital status" instead.

Neither of the consent checkboxes currently exists. In lieu of "Consent for HSA to check Citizenship in federal hub," I'll substitute the "U.S. Citizen or National?" field for now.

The last two fields don't exist in the PDF form with which I've been working, so I'm going to call it a night and find out whether they exist in some other form. (Literally.)

Again, those fields are: 1) "Consent for HSA to check income in federal hub" and 2) "If elderly (65+) or disabled, then requires verification of resources."
