Add support for Koa@1 #1

Hey,

Thanks for this awesome package! I find myself enabling Koa middleware based on a request condition a lot and koa-if would make it a breeze. But the lack of support for koa@1 makes it impossible for me to use it. Have you considered providing support for koa@1, if so, are you adding it yourself? If not, would you mind me doing a PR?
