Need support for NCX navigation #507

Hey @fchasen , As per comments on pageList.js, epub.js doesn't support NCX for purposes of pagination. Any specific reason for the not supporting NCX format. If not for pagination, can epub.js support TOC navigation from NCX format.

Just never implemented, focus of the library is Epub 3 but would be great to support NCX.

hope can support render toc tree by ncx when it is exists, because opf not support tree level

ncx can make toc is looks better
