Adding forums doesn't appear to work #188

Adding forums with the AJAX forum editor doesn't seem to work. When a forum is added, it says "Saved!" but when the page refreshes the forum isn't there.

It appears the issue is with collecting the form data in the collectFormData function. Everything is being sent in undefined.
