I have had cases where the 2nd ad did not get muted.
I've noticed this happening occasionally, but recently it has been more reliable for me than in the past... To be honest I've never been annoyed enough to look into how to fix it. Especially since it's not a consistent issue making it hard to pinpoint a fix. I may try to put some preventative measures in place and see if it makes a difference.
I have had cases where I have to press play to get the video playing after the ad has been skipped.
I have never experienced this one. What browser/userscript manager do you use? I've mostly used safari with the aptly named "userscripts" extension, and Firefox with violent monkey.

I am using Tampermonkey on OperaGX.
Is there a diagnostic log thing I can send here that can help you in diagnosing the issue?
Update: I may have a lead on the having to press play for the video to play, I think it has something to do with changing video and removing old wait on the console log and waiting for ads.
I have been experiencing various issues with the userscript when using it.
My settings: Mute ads: Checked Hide video ads: Checked Banner ad close delay (seconds): 0 Video ad skip delay (seconds): 0 Don't skip ad shorter than this (seconds) : 0 Mute video even if not skipping: Checked Show extra debug information: Checked
Is there a recommended settings or is there something that I should look out from the console?
