DICOMAligner incorrect for non-perpendicular orientations #200

For images where the image axes are not perpendicular to the LPH reference 
frame, the aligned image
a) has the wrong offset (origin)
b) is often cropped at the corners 
c) has a spacing in which data is lost (decrease in number of image voxels)
