Add nchild's note to DB & result page

Please run DB migration and run import script after pulling in order to populate your local database's categories table with notes.

ex:

sequel config/database.yml -m migrations
bundle exec ruby scripts/set-note.rb pod/result-notes.v1.1-utf8.csv

那個 csv 裡面的 Category 是不包含在那份 PDF 裡面，但是未來也會開放的 Category 嗎 /to @nchild @MrOrz
