Updated and reorganized explanatory text based on PRC comments
Updated examples to align with updated Phenopacket implementation guide
Changed raceEthnicityAncetry to populationDescriptors
Remove Pedigree.reason as it isn't used anymore

@lindsmith @liberaliscomputing Sorry for the delay in getting to this. Here is a first pass of updates to the docs for the PRC's upcoming meeting. If the meeting is today, we can just merge these changes so they have something improved to review, and then people can iterate and improve them.

@buske The PRC is meeting today, in just under an hour (9am EST). I'm happy for us to merge, but no problem if we don't do it prior to the meeting. I can share this PR instead.
