Just displaying a number on top of the Comment Badge is really not an obvious enough method to signal that comments have been left on an URL using Dissenter. Sometimes I don't even notice that the icon has changed.
Feature suggestion: make an (optional!) feature that will, when Comment Badge is enabled, display a small animated popup under the Dissenter Comment Badge (which is to the right of the Firefox address bar) whenever you visit an URL that has Dissenter Comments.
Because it will be an animated popup it will be way harder to miss.
EDIT: ALTERNATIVE: Make the icon flash/blink (animated, repeating) when there has been a comment detected.
Also add an ignore list so we won't get the popup notification when visiting the main page of Google.com for example.
