can confirm this issue exist, noticed it too in sunwell at twins, sacrolash hits only with main hand. looks like global issue with NPCs that have dual wield weapons.
I also noticed this problem. It applies to all mobs and bosses who have 2 weapons in their hands
If errors that directly affect the damage per second of the boss receive the Low Priority status, then is it worth reporting problems of the same kind? About the incorrect speed of automatic attacks from raid bosses? Or will they also receive a low priority and be postponed indefinitely?

Description: The bug is that NPCs equipped with two weapons (or hitting with two hands) hit with only one main hand. This is a global problem for all NPCs living in the BC and WOTLK dungeons. When I was rocking the character, I noticed it. The mechanics were added in patch 2.5.1. The damage from the left hand should be 50% of the damage from the right hand. Also, the dual weapon mechanics must be accompanied by a passive skill +20% hit rating
