When an expense is approved by an approver the notification email is not sent

Make sure multi level expense approvals are enabled
Add an approver to the user (who is not an admin, a manager or a supervisor of the employee applying expense)
Apply for an expanse as an employee
Approve it as the manager
Try approving it as the first approver
