New user agreement not showing up

What steps will reproduce the problem?
1. Going to New User Registration Module
2. Adding text in New User Agreement Message
3. Saving configuration
4. Viewing new user registration page

What is the expected output? What do you see instead?
The expected output is a User Agreement page
I dont see one.

What version of PWM are you using?
1.6.4

What ldap directory and version are you using?
OpenLDAP

Please paste any error log messages below:

Could it have anything to do with this comment in the Admin Guide?

"Note that using any other path than /pwm will not work, at least out of the 
box. For example, submitting the CAPTCHA form will fail with "Form submitted 
with incorrect pwmFormID value" error."
