Remove references to LOADED_PACKAGES #18

This removes some dead code, and cleans up FillPoints a little bit.

With this patch, grepping for "LOADED_PACKAGES" (when searching for packages using obsolete function) does not anymore show guava as a false hit.

Thanks for the cleanup! I didn't think it through and only commented-out the uses of LOADED_PACKAGES so while it wasn't being used it did show up in a grep!

BTW the CreateBoundsTable function is not in a rational state; the bounds tables have apparently been hand-edited rather than created using that function and there are many inconsistencies.
