Add Issue Template for Release Responsible tasks for a release #796

What would you like to be added:

We need to add issue template for the activity

An issue with this template should be created per milestone and release responsible should be assigned that issue.

Why is this needed:

Today when we release a minor version MCM , there are many tasks which should be performed before and after the release. This issue template will have a checklist of all such tasks.
