Credit limits

The credit limit should be displayed as an extra column on the billing summary screen (mockup #10). Diners should not be able to sign up for more meals when they reach their credit limit.

This ought to be discussed with the various communities meals committees - particularly around renters, as there's differing opinions on the business rules around this.

Once account data is migrated, we need to get rid of the over_limit field and compute it for real
