StExBar installation causes Windows Explorer's Menu bar to always show #189

What steps will reproduce the problem?
1. Download, install and activate StExBar
2. Open a new Windows Explorer

What is the expected output? What do you see instead?
A Menu bar is always shown, irrespective the settings under Organize | Layout | 
Menu bar.

What version of the product are you using? On what operating system?
StExBar 1.8.3.352 (first and fresh install) on Windows 7 Enterprise SP1 32-bit.

Please provide any additional information below.

I really wish to hide the Menu bar again, so I tried the following, in the 
order mentioned:
- hide the StExBar: hid the StExBar but did not help for the Menu bar, even 
when killing explorer&IE then reopen Windows Explorer;
- disable the StExBar via IE | Tools | Manage add-ons: did not help, even when 
killing explorer&IE;
- uninstall the StExBar via Control Panel: did not help, even when killing 
explorer&IE;
- removing the Enum registry entries mentioned on 
http://tools.tortoisesvn.net/StExBar.html: did not help, even when killing 
explorer&IE;
- renaming the %ProgramFiles%\StExBar\StExBar.dll file (gave .dll_ extension): 
did not help, even when killing explorer&IE;
- restarting my machine, menu bar is still there, unremovable!!

Please provide me with proper repair instructions ASAP.

Dug some more, menu bar issue was related to a corrupt toolbar (not StExBar!) 
which was indicated as hidden (unchecked toolbar) but was in fact only 
invisible instead of logically hidden (due to the corrupt state of the 
toolbar). When all toolbars are hidden, the menu bar could be hidden as well :]

Now here is a question: is it true that as soon as any toolbar is active and 
shown in Windows Explorer, the menu bar is also shown, always? (Otherwise I 
still have an issue with the StExBar)

at least on Win7, if a toolbar is shown the menu is also active.
