Make allergies / dietary restrictions report #164

Dependent on #163 happening first

Make a report for dietary restrictions so we don't have to hunt thru each family searching for any allergies or dietary restrictions.

Make report show only ones for people registered for current year.
Put person's name, and the value in the text field.
