Show list of people who still have registration items due #23

As the date of the event approaches, the registrar needs to follow up with people who have outstanding items to turn in (health form, release of liability form). Each participant must turn in these forms, so indicators of whether they have turned them in should be on the participant.

The system should allow the registrar to get a list of people who still need to turn in particular items, and show what those items are. That list will be a new report under the "reports" menu.

added health and liability fields to participants, and added editing for them to the _form and the index table for participants.

Working on creating a report that will have these items listed.

re-thinking use of these fields. They're not used in release 1. Would probably still be nice to consider for Release 2.
