Report start-say / start-play without lag #32

There can be a sizable delay between a callback indicating say / play start
and the actual start of audio output. The extensions current send start
immediately when processing the say/play command, not when output actually
happens. Change this behavior so the start occurs as output starts. Perhaps
also add a new notification name to represent what we currently report as a
start.
