One dead outfox process prevents others from launching #37

When an outfox process throws an exception, it becomes a zombie. Further
messages from the extension are ignored. Because it never dies, the
extension does not try to launch new processes for other pages.

Fix the logic in the extension so one dead process does not block future
processes.
