Moved code from telescope plugin to a regular module
Cleaned up:
- Code actions don't fit the rest, and there are much better handlers out there. E.g. https://github.com/weilbith/nvim-code-action-menu
- Removed handling of ctx as a second result dataset
- Fixed encoding warnings
- Removed key mappings routines, leaving that to telescope
Added some basic code comments
Simplified options passing, Pickers.new now has weird defaults provided by me and will accept sane options provided by the user

This resolves https://github.com/gbrlsnchs/telescope-lsp-handlers.nvim/issues/6 gordian knot style.

This PR changes a lot of things, and these changes will totally break this plugin for folks.

Also, as I'm not using Telescope anymore, I don't feel comfortable of blindly merging this, as I wouldn't be able to validate the changes.

I see you have nice ideas for the plugin, even updating the docs. Would you be willing to maintain a fork of this repo? I'd gladly point your fork in the README, if that would be the case.

I wouldn't use "maintain" in relation to myself, as I'm unlikely to react to feature requests or issues with anything resembling haste. But I'm gonna keep it working for as long as I have a use for it.
