relatedcontent on stories should have constraint #68

as is on the story model the same object can be related to a story more than 
one.  We probably 
want to create a unique key constraint on the relatedcontent model so this is 
not possible, there is 
no reason I can forsee the same object needs to be related to a story more than 
once.  

This came up while fixing a bug in editing stories media. Media was being 
re-created rather than 
edited/saved.
