This is not really and issue for A2DP Volume. It seems like you just need to
disconnect your headset before you start your car. You can also use A2DP
Connect widget to force an A2DP connection with the car when you forget your
headset.

Everybody seems to point me to yet another app saying it will fix my problem
and none ever do.
I know I need to disconnect the headset before I start my car. This is the
problem I'm trying to overcome. I forget. It's not like everytime I walk up to
the car I'm currently listening to some audio on my headset. Sometimes my phone
is in my pocket and my headset has been on quite literally for hours but I have
not be actively listening to anything through it.
Let me ask you, how do you get the A2DP Connect widget to force an A2DP
connections with the car when I forget my headset? Because AFAICT this will
never work. There is some sort of a bug in the Bluetooth driver I guess in the
car that once this condition is hit you cannot make A2DP connect from the cell
phone to the car until and unless the radio software is reinitialized. Think
about it - if it was just a matter of "forcing an A2DP connection with the car"
then why wouldn't simply going to the Bluetooth settings and say "Connect A2DP
to the car" work? It doesn't work. That's the bug and AFAICT it's a bug in the
Bluetooth implementation in the car. No other bluetooth device that I connect
to has this odd behavior. If I cannot do it manually then how or why would this
A2DP Connect widget be able to?!?

A2DP connect2 will not solve this problem either. Thanks for answering my
questions.
