Add documentation / templates for allowing the apache to blacklist in cases of unintentional DoS #530

In some cases we may need to shut off certain IP addresses / hosts that are causing temporary problems. This ticket is closed with either documentation or or other means of allowing this to happen in an AmiGO production environment.

@stuartmiyasato I thought you might find this interesting for your own stuff, though you may do it differently. I've tried this out and it works great. As apache seems to pick up the list hot, it would be very easy to make a real-time add/back-off system.
