Multiple options missing from + icon at right side of lines in CC form #228

Multiple options ("Search annotations", "NOT qualifier", "add an Extension >", "Evidence >", "Add Root Term", and "Clear Values") are missing from the pull down menu that appears if you click the "+" icon at right side of lines in the CC form. Instead the pull down menu now offers term relationship options.

@LiNiMGI please add comments if I missed anything you wanted to report.

Compare with these two forms, where the pull down menu shows options for Evidence: BP only form BPform

@krchristie We discussed this one on today's workbenches call, too. The 'three dots' menu disappeared from the CC only form.
