Add standard annotation form icon to the 'Open in' list of workbench icons on the Noctua landing page

Curators will want to be able to use the icons menu to the right of models on the Noctua landing page to readily access their models.

We'll need to select an icon and add it to the menu.

For now, we can just add another form-like icon. Eventually, we will retire the existing form and associated icon.

@tmushayahama For the testing period on noctua-dev, we need to keep the icon for the current noctua form, so we will need to add a new icon and distinguish it from the old somehow (color?). Once curators have tested and we're ready to move to production, we will just have one form icon on production.

Icons exist now to the left of each model in the table, and a link to the Standard Annotation Editor is also available in the Open in list to the right of the models in the table.
