Add automatic linking #43

Post titles and notes are now automatically linked using Linkify. Links are inserted on the client end using JavaScript.

Yes, it looks very ugly right now, but I'm still working on design and layout.

Titles will no longer be automatically linked on the post listing page, because they would link to both the post page and the destination of the user-defined link.
