Ryan,

This patch does the following:

Correctly output messages from server to client (should fix why edits did not get applied)
Adds logging of server -> client messages
Fixes 2 memory leaks
VS Code requires a version number for the documents in documentChanges, so 0 is passed.

With this patch and the changed extension, code completion works in VS Code.
